Compliance Standard
Willingness to participate in HOPE's program requirements, case management, and accountability standards throughout the transitional housing stay.

Willingness to work toward employment, financial stability, and independent housing with support from HOPE case management.
Selection Protocol
Placement is subject to housing availability, background and safety review, supervision requirements, and program eligibility. Submitting a referral does not guarantee acceptance or placement.
How the Referral Process Works
Initiate Application
Call 470-598-4673 or submit the online referral form to begin the intake process.
Eligibility Screening
Our team reviews program eligibility, supervision status, and housing needs to determine fit.
Housing Match
If eligible, we identify available housing based on program capacity, location, and safety review.
Transition Planning
Approved referrals begin individualized transition planning with a HOPE case manager.

Required Client Records
- Government-issued identification; relevant case or court documentation; supervision or release guidelines; income and employment information; and any behavioral health documentation, where applicable.
